Do you have a young baby in hand and you need to go out with her/him?
May be the below things should help you in packing stuffs for the baby. This is what I do when I pack up things for my new born baby girl.. Hope this helps.
1. Have a comfortable carry bag to have all the things in here
2. Take 2 diapers if your outing is less than 5 hrs of time
3. Take 2 bottles of expressed milk or formula milk. Let the formula milk be luke warm. You can also store this milk in the bottle temperature maintaining kit
4. Have a spare dress for your baby just incase she regurgitates quite often and wets her dress
5. Take dry tissues - I generally take the whole box instead of stripping few tissues from the box
6. Take wet tissues - If the baby poops in the diaper during your travel, you cannot search for water everywhere. Wet tissues do the trick. Again, I take the whole box
7. Pacifier/soother - I consider this to be very important simply because not everyone around is tolerant if babies cry. To keep the baby pacified and also make the environment not look at us when she cries her lungs out, pacifier will be of first hand help
8. Rattler - Just to play with the baby if she gets uncomfortable in the new place
9. Nappy rash cream - When you change the diapers
These are what I carry whenever I go out for a short period with my baby.

Hey Lakshmi.. COnsult with your doctor before using baby powder after diaper change. For baby girls especially there is a study that it might lead to some infections.....

Do not use baby powder containing talc, especially with girls. Talc can get into the bloodstream through tiny blood vessels in the vagina and cause problems for a baby later on in life. There are great baby powders that only contain cornstarch, and these are better for using on the bottom, so read the label.

None... My Doc is quite against using baby powders! I use vaseline and baby lotion after every diaper change!
Oh ok! I also use nappy rash cream and then apply powder for her every time I change her diapers.
